All referrers will be asked to complete an Evaluation Form following the approved request, and once the item(s) have been purchased and received by the individual / family. This will assist us with reporting to the Awhi Community Fund donors on the allocation funding together with individual impact stories.
The Evaluation Form is made up of five questions with an option to add further information or photos.
